ACCEPTANCE OF TERMS By booking an appointment or using any service at Modality Skinn, you agree to these Terms of Service.
SERVICES Modality Skinn offers professional aesthetic and wellness services including: skin lightening, body contouring, facials, body waxing, brow lamination, eyelash lifts and tints, eyelash extensions, seaweed and mud body wraps, body polishes, ice body sculpting, and Vitamin C shower treatments, Cold Plunge, and Sauna.
HEALTH DISCLOSURE You are required to disclose any known allergies, skin conditions, medications, medical history, or health conditions prior to your appointment. Failure to disclose relevant health information releases Modality Skinn from liability for any adverse reactions or outcomes.
RIGHT TO REFUSE SERVICE Modality Skinn reserves the right to refuse or discontinue service to any client at our discretion.
CANCELLATION POLICY Appointments canceled less than 24 hours in advance will be charged 50% of the scheduled service fee.
NO-SHOW POLICY Clients who do not arrive without prior notice will be charged 100% of the scheduled service fee.
LIABILITY WAIVER By receiving services, you acknowledge that aesthetic treatments carry inherent risks including skin sensitivity, redness, or temporary discomfort. Modality Skinn is not liable for adverse reactions resulting from undisclosed health conditions or failure to follow post-treatment care instructions.
PHOTO & TESTIMONIAL CONSENT Modality Skinn may request permission to photograph before and after results for promotional purposes. Your consent will always be obtained separately and in writing.
GIFT CARDS & PACKAGES Gift cards and prepaid packages are non-refundable and have no cash value. They may be transferred to another person.
PRICING All prices are subject to change without notice.
PRIVACY Your personal and health information is handled in accordance with our Privacy Policy at modalityskinn.com/privacy-policy.
GOVERNING LAW These Terms are governed by the laws of the State of Arizona. Disputes shall be resolved in Maricopa County, Arizona.
UPDATES TO THESE TERMS We may update these Terms at any time. Continued use of our services constitutes acceptance of any updated terms.
